You're reading One flew over the chuckoo's nest by Ken Kesey, my question is what do you think is the significance of the title? My take is that a person(Randle McMurphy) takes over and tries to overthrow the person in charge(Big Nurse) and tries to make a change.

I agree with much of your answer except the bit about McMurphy. I feel that he did go crazy in his own way. Although he rebelled against the Big Nurse and the Combine he ultimately submitted to them by doing what was expected of him. He had no choice in his actions after Billy's death.
