I do avoid it, but it's due to the fact that my game is weak and my use of the lesser pieces is poor. I need my queen!

It depends. If the rest of my pieces are quite active, then an exchange of queens improves my position. I will also exchange queens with players like MrWitch, who admit they are not as strong with the minor pieces as when they have their queen. I consider myself quite good with the minor pieces and rooks.

That depends on how good my opponent is realtive to myself. I assume a player with a higher rating than myself will benefit from a complicated postion / more peices on the board. They are probably able to calcualte better than me, so in league matches - if I am playing soemone who is rated much higher than myself then I want to simplify the game as quickly as possible (I will be happy with a draw) so swapping Queens and for that matter any of my pieces is a good idea. Against a weaker player - I assume the more complicated the position the better for me. I also take much greater risks against a better player. Its the only way I am likely to get a chance of winning.
