The testes (medically, 'testicles' is a diminutive term for a boy's testes) are examined for masses that could indicate cancer or less serious conditions. The 'turn your head and cough' routine is to check for inguinal hernias. The physician pushes the scrotum up into a natural space, the inguinal canal, that approaches the inner abdominal cavity. This is the passage through which the testicles descend from inside the abdomen during fetal development to enter the scrotum. If there is a hernia, the doctor can feel bulging in the canal when the patient bears down while holding his breath, which temporarily raises the pressure in the abdomen. A milder version of this is coughing. Turning your head is so you don't cough on the doctor!

I have a female doctor and she's never done that test on me. I feel ripped off. But dont want to ask her! And she does the prostate exam so quickly, I hardly get a moment to enjoy her finger in me! What a rip off! How about putting me on my back and in those stirrups? What is with these female doctors - I have fantasies, ya know! She's not fulfilling them at all! Part serious, part kidding. Your question is legit, I've always wondered too.

I am castrated. I've been examined by women doctors who've asked why I have my testicles removed. I find them very professional.
