    I'm guessing it's an American thing for calling some one "a bit nutty". It's probably only a recent thing (last ten years), but the meaning is the same. A few slices short of a loaf for example. I'm Australian though so I say "A few beers short of a carton". Cheers big ears

    It was from stargate sg-1, because im a true pure stargate fan. It was from episode window of opportunity.
    it was kinda like a groundhog day episode. awesome and funny though. Also, it means crazy, like how Colonel Jack O'Neill said it. Here is the lines in the script.

    TEAL'C : O’NEILL, should we not be assisting DANIEL JACKSON with the translation?

    O’NEILL starts to draw on the plate with the ketchup and mustard.

    O’NEILL : I’m taking this loop off.

    TEAL’C considers this and sits down.

    O’NEILL : I’m telling you TEAL'C. If we don’t find a way out of this soon I’m gonna lose it.

    TEAL’C stares at him.

    O’NEILL : Lose it…it meant go crazy, nuts, insane, bonzo, no longer in possession of one’s faculties, (he continues drawing on the plate with the condiments) three fires short of a happy meal…WACKO!

    He lifts up the plate to TEAL'C and we see that he has drawn a smiling face.

    The phrase came from the line delivered by the character Col. Jack O'Neill in the episode "Window of Opportunity" of the sci-fi show, STARGATE. And it meant someone not all together there, crazy, or going bonkers.

    omg! people have been saying it long before stargate sg1!!! omg whoever thinks that the writers of that show came up with it are total losers. they also said it on supernatural...oooh maybe mcg came up with it. get over it. Hey guess what.. my boyfriend made it up like 20 years ago..try that one on for size..freakshows...
