For insurance that says they cover something 100% after a $500 deductible, what exactly does that mean? For example, for each doctor appt. would I pay the full office visit cost, until it reaches $500?

I'm not 100% sure that the office visit fee counts towards the deductible. Does the insurance company make you pay an office visit fee (usually like $25 or more per visit)? My insurance plan is similar to yours, and I think I pay office fees for each visit, no matter how many times I've gone to the doctor. The fees above that would count towards your deductible (any surgeries, any extras, etc). You will have to read the fine print in your health insurance booklet. It should tell you, in detail, what goes towards your deductible. If not, just call the customer service number and ask. :)
