Help answer this question below.

My 7 year old chiuaua was diagnosed with a heart murmur one week ago , i took him back to the vet today because he was in severe pain mostly in the evening, with his neck swollen , back arched, cant even touch him, he hurts so bad. what are the causes?

Didn't the vet lay out the causes and treatment plan? He'd certainly be the most qualified professional.
