Sometimes they do, but only in the higher quality movies. In low budget films they aren't that concerned.
Also, there is likely to be a mixture of bodily fluids, lubricant, and sweat down there that would probably smear the makeup around or turn it into an unpleasant looking brown liquid.

If you can tolerate the ugliness of the whole situation with the mess and moaning and grotesque looking parts being shoved in your face I guess they feel a little acne won't bother you!
