They usually do shoes sizes for children that young by age in months, or a have guide to tell you what size you should buy. Also, a lot of kids' shoes come in stretchy material. Ask the length of the child's foot - http://www.sizes.com/people/shoes_infant.htm and look there.

About a size 4, at least that what my sons about to wear. He's growing out of his size 3 shoes now.
