Very, very rarely. Pilots are governed by laws controlling how low they can fly in relation to built-up areas, so that in event of trouble they should always be able to steer well clear of buildings and people if they have any control at all. Rather surprisingly, the rate of survival of even the occupants of small planes which crash is much better than you might think. Which is why people no longer wear parachutes. You have a much better chance staying with the plane than trying to get out. This is because these little aeroplanes crumple easily, absorbing the impact energy, and in particular they slow down greatly if the wings are ripped off. One important thing is to get out quickly in case of fire. One of the items in the pre-crash checklist is to unlatch the doors and get them slightly open so that they cannot jam shut if the frame distorts.
