You can create a fillable PDF using Adobe LiveCycle Designer ES, available with certain versions of Adobe Acrobat, from scratch or from a template. In the Adobe Acrobat menu, select "Forms," "Start Form Wizard..." and "No Existing Form." Click the "Next" button to open LiveCycle Designer, and follow the wizard instructions to create a fillable PDF in that program.
Convert an existing electronic document or a scanned paper document into an interactive PDF file. From the Acrobat menu, select "Forms" and click "Start Form Wizard..."; then click either the "An Existing Electronic Document" radio button to convert an electronic file or the "A Paper Form" radio button to scan a paper document and convert it to PDF. Click "Next" and follow the wizard instructions.
You can convert an existing PDF document to an interactive PDF by selecting "Forms" and clicking "Add or Edit Fields..." in Acrobat if you do not want to use the wizard for conversion.
Add interactive fields to the PDF, such as text boxes, drop-down boxes and check boxes. Select "Forms" and click "Add or Edit Fields..." from the menu. Right-click the PDF and choose an interactive field to add; alternatively, select "Forms" and "Form Tools" and then choose a field.
