Use the federal 1040X form to amend your original tax return. You will enter your personal information in the first section as it appears on your original tax return. Check the appropriate filing status and check the box for the year tax return that you are amending. Explain the reason in detail why you are amending your tax return.
Enter the correct amount in the column for each line under the income section if you have a change to your adjusted gross income. If you are changing the exemptions, you will need to use the worksheet on page 2 of the 1040X form. Make any changes to the tax liability and payments if you have any. Remember that if you change the adjusted gross income, you may need to change the tax liability and payments sections.
Calculate the amount that you owe or that you need refunded in the last section. Sign and date the 1040X on page 2. Send the amended return to the correct address for your state found on page 5 in the instruction booklet. Note: If you change your adjusted gross income, you will need to amend your state tax return.
