If you mean how do you know if someone has hacked your computer then there would be quite a few things that could have been done. Probably the most common is putting Malware, Adware, Spyware or some other form of virus that can slow your computer's performance down to almost unusable. They can also steal your personal information by logging what you type on the internet and elseware. So if you notice any signs of your pc running really slow or unrecognized programs then you'll know.

You don't hack an IP address. You hack a computer, or a server. The IP address is just a (usually temporary) identifier for the computer.
