I think consistency would be best. There is usually a space for "other" and if you are a wonderful mix of ethnicities I would list them all. I don't know about legally obligated but if there were any instance where forms/applications you have submitted were compared and there were inconsistencies you might be called down for that. I wouldn't take the chance. It could lead people to believe you were hiding something! Happy Tuesday! :)

In almost every document I've ever filled out that asked about my race/ethnicity, the section about race/ethnicity is labeled "Optional".

I believe it's against the law to require someone to divulge such information.

I am not entirely sure whether you are legally obliged to provide ANY information about race - only about nationality. Many ethnic monitoring surveys include a sentence to say that completion is voluntary.
Ethnic monitoring surveys are designed to monitor the ethicity of a company, or a service to ensure that a mix of people are employed, or receive the services. It does not form an integral part of a job application, or a request for a service.
For example, I never complete the ethnic monitoring surveys at my local hospital, as i would rather receive the treatment that I need rather than the treatment that is appropriate for someone of my ethnic background.

if the information is false, race is not something they will fire you for off that back ground check.
