A health risk assessment is a screening tool designed to help identify your major health risk factors. They are offered in several different settings. For example, your physician, employer, insurance company or even community may offer them. They are commonly offered at local wellness fairs but can also be found online through a number of providers.

Wellness Fairs

Go to a wellness fair. Almost every community hospital hosts an annual wellness fair. These are typically events designed to promote awareness about wellness and health care delivery. Nurses and health educators usually offer health risk assessments during these fairs to help you update your knowledge of your overall health risks and behaviors.

Employers

Talk to your human resource representative. Depending on where your work and how many people your company employs, you may be able to update your health risk assessment while at work. Companies frequently have an annual screening or health awareness day and offer assessment updates at that time.

Doctor's Office

Check with your primary care physician. When you go in for your annual physical examination, ask your doctor. The typical clinic physician sees lots of patients every week and it may not occur to him to update your health risk assessment. Ask if you can take a health risk assessment in your doctor's office.

Online Assessments

Several organizations offer online health risk assessments. The Mayo Clinic, Kaiser Permanente and Blue Cross/Blue Shield are examples of groups that offer comprehensive online assessment tools.
