  • A health risk assessment is a screening tool designed to help identify your major health risk factors. They are offered in several different settings. For example, your physician, employer, insurance company or even community may offer them. They are commonly offered at local wellness fairs but can also be found online through a number of providers.

    Wellness Fairs

The typical clinic physician sees lots of patients every week and it may not occur to him to update your health risk assessment. Ask if you can take a health risk assessment in your doctor's office.

    Online Assessments

    Several organizations offer online health risk assessments. The Mayo Clinic, Kaiser Permanente and Blue Cross/Blue Shield are examples of groups that offer comprehensive online assessment tools.
