The movie that has scared me the most, for the longest amount of my life, is
"Night of the Living Dead" ( 1968, original )
"Avatar". That piece of cinema made me vomit.

Depends on how desensetised both of you are, not much scares me, but Japanese horrors get the closest, in my opinion, the original ju-on (grudge) is the best though

ju-on, the original the 'grudge' film. freaking scary. the only movie that makes me fear dark places. seriously.
Well since you have posted this under '"Slasher and Splatter movies" first a few genre classics: Halloween, Nightmare on Elmstreet, Hellraiser, Friday the 13th, Texas Chainsaw Massacre, they all are great movies and for most people very scary.
Scary movies of other genres would be for me, first some classics:
The Last House on the Left (NOT the remake!!), The Omen, Rosemary's Baby, The Excorcist, the Hills Have Eyes.
A Little more recent:
Wolfcreek (Very brutal and realistic, at times hard to watch), Funny Games (the original!)
And some Asian jewels, hardly any gore but really creepy:
Shutter, Ringu, Dark Water, Ju-on, Noroi, Kairo, The Eye, ...
Well... it depends... you have to know what scares her... like me for example things that are realistic dont do anything. Like maniacs and serial killers don't scare me because they are human and something I am very much aware of and have an idea how to handle. However irrealistic movies such as "Omen" or "Mirrors" and stuff like that, freak me out, because I am scared of things that seem mysterious and out of my control what's so ever. However I don't take "Final Destination" all that serious at all either.
SO here goes my list in no particular order of importance:
Halloween (yes I know realistic but the sounds effects are good and keep me jumping)
Number 23 (For some reason I found it scary... don't even know why...)
Crazies (But I watched it in cinema...so that could have been the reason... althought the sounds are very jumpy too)
Mirrors (I freaked at my reflection for quite awhile after it... still get goose bumps sometimes)
13th Ghosts (When loud... really freak me out... don't know why either, the sounds are creepy)
Omen (I find the idea scary for some reason)
and... yeah well... Paranormal Activity (I have to admit it's a crappy movie ... you will be disappointed... but if you like suspence and waiting with adrenaline boiling...then it might be good for you)
But to be honnest and frank... any movie (almost) can be made scary if in the right setting... make sure to turn off the light... leave a window open (the wind sometimes gives ppl creeps) ... and well she will have to be also in the mood for it... otherwise you could laugh at the scariest movie...or simply watch it like whatever...

I liked 13 ghosts and the possession of Emily something or other? I forget the exact name. I woke up every night at 2:00 am for weeks after that movie and sat in bed for an hour freaking out.

I remember sneaking into see Jaws when it first showed in our local movie theatre, we were all too young and that was scary at the time. Alien was pretty scary too and the end of The Blair Witch Project when they find the old house in the woods. We lived in the stixx when we were growing up and there was a woods nearby and it had an abandoned house in it. It used to scare the crap out of us when it got dark early in Winter time.
The Ring
It's an oldie but it scared the bejesus out of me: "Rosemary's Baby." I saw in in the early 70s, I think, and I can still remember scenes from it.
Paranormal Entity. Live Footage, however, it might be tooo scary, and scare the hell out of your GF. YOU HAVE BEEN WORN. If i were you, do some research about the movie.
If she hasn't seen "Jeepers Creepers" ...She'll be in your lap 1/2 way through the move!
You know I like a good horror movie but they don't REALLY scare me.Some of them are actually pretty funny.But your question reminded me of something that I experienced that I heard had happened only once before with The Exorcist.I remember when the first Nightmare on Elm Street came out.You know there's a whole thing now with Freddy everybody loves him.But when the first, original movie came out people were actually getting up and leaving the theatre.They could'nt sit through it.I actually saw this happen.People were disturbed by it. It is a good movie. I love Wes Craven. But it's interesting to watch society progress.How it gets used to things.I know older people who still admit to being terrified of the Wolfman and The Mummy. We laugh now, but I notice when something is new it's shocking to people but after awhile people get get used to it and it takes more and more to get that reaction.But I liked The Grudge1, if you haven't already seen it.
The movie REC and REC2 really got my going Check it out.

The first "Faces of Death" For some reason I was ill halfway through this film,somewhere around the live electric chair demo.
This one documentary that went over all of the brutal experiments done to people around WWII. They shoved life cockraches and fleas into people.
Suddenly last summer!
