When does the President not salute the troops or put his hand over his heart?

    Most of the time.

    The President (of the USA, I presume you mean) only salutes troops when taking a parade, the same as would any other reviewing officer. As for the peculiar American habit of placing ones hand on ones heart, this is the equivalent of saluting, so should only be done where military personnel would salute, though usually only on parades or for national anthems.

    Usually only persons in uniform salute or return salutes. Putting a hand over your heart is usually done for the national anthem or pledge.
