A guy is always aware of his penis. That's not to say he dwells on these thoughts.
It's among the more vulnerable parts of his body, along with his testicles, so he learns from a young age to protect it.
Having knowledge of this vulnerability, it makes me laugh when certain feminists say that a man uses his penis as a "weapon".

Gotta take a leak? You'd better make sure you know where it's pointed or it'll get messy.
Taking a bath or shower? It gets special attention. There are lots of folds and surfaces that need to be clean. There are lots of stinky things that happen down there, you need to pay attention to cleanliness.
