Miss is always when someone is unmarried. Mrs. is always when someone is married.
Ms. is used when a woman doesn't want to be called Miss or Mrs. -- or when you address a woman and don't know her marital status.
I'm married ... and use Mrs. in personal things -- but might decide to use Ms. in business related situations where my marital status plays no role.
