For my physicals, I am barefoot yes and although not naked from the start end up that way. When I go in the exam room I have to strip off everything except for my boxers. The nurse takes my weight and height before taking skinfold measurements to make sure my body mass is ok. She then takes my blood pressure and carries out an eye and ear exam, as well as watching me give a urine sample. I'll then have the nurse go out and the doc come in. He checks my reflexes and examines my back by me leaning over and touching my toes. He then steths my chest back and stomach before I lie down on the exam table and have my stomach palpated. I then have to stand up and remove my boxers so he can examine my dick. I have to turn and cough for a hernia check before he pulls my foreskin back to make sure my head is ok and then feels my shaft and balls. I then have to lean over the exam table while he checks my ass. I then have some shots and can then get dressed and leave

No of course not. The doctor will only tell you to drop your pants when he checks for a hernia, and that's only for a few seconds. Otherwise, you have all your clothes on the entire time.
