typically the signs are water/coolant in the oil, oil in the coolant, a miss. a lot of times you can tell by just pulling the plugs and looking at them, but the best way is to run a compression test.

There is a tool called a block tester, it uses a blue chemical that changes color if exhaust gasses are present in the cooling system. Get one at NAPA for around 40-50 dollars I think. Very accurate when used properly
