I think the offense, is found when people call Indian's, Pakis because Indians and Pakistanis traditionally don't get along... It's like calling a Scottish person English, They would NOT be happy, and probably take offense.

I quite agree that an Indian Asian might take slight offence against someone calling him a Paki, not because he thinks it is an insult, but because he is an Indian The word Paki is an abbreviation of Pakistani and its only the politically correct brigade who object to the word. If Pakistani’s were white skinned I wonder if they would object then?
