I am in the UK, and where I live it's usually toilet or loo. I prefer lavatory, though.
New York- the toilet, john, poopenschlammer, place where you go to put one through the hoop, drop a deuce or take the Browns to the Super Bowl.

I was taught not to mention it at all if possible. So when you knew where the facilities were located you simply said, "Excuse me"
I you need directions you could discretely inquire where the "ladies room" was.

In Singapore, its usually "washroom"
In Kentucky we call it the stink swallower
I live in the UK. Most places i go out just have a sign saying 'toilets' and an arrow pointing the direction or something.
Toilet is seen as a polite word here and most common really.
Australia - the toilet or the bathroom
In public you'd say you needed to go to the Ladies or Gents, depending on your gender. At home you'd say Bathroom. Or in either case, just toilet.

In my house a bathroom has a shower or bathtub in it, a restroom is only toilets & sinks.

Southern California - The Bathroom or the Rest Room
The most polite term would probably be the ladies' room or mens' room. But washroom or bathroom are more common and they're considered polite. And there's plenty of other things people say, like facilities, toilet, loo isn't rude but I'd only say it in casual company.

In normal conversation I usually say "bathroom," but if the situation calls for a bit more formality, I use "rest room," "wash room," or "mens/ladies room."

That would be the Restroom - Central California.
We use the word "Restroom" around here. Here is just outside Atlanta, Georgia, United States of America.
