It's a self confidence issue. If you approach a girl and are NOT confident about yourself, they will see it and be rejecting. Every girl wants a confident man. About the fear of rejection, that is just something a little practice will get you over. I'm not saying to hit on every woman you see, but strike up friendly conversation anytime the opportunity arises. I tend to find great opportunities to practice this at grocery stores, gas stations, libraries, church, and coffee shops.
