I guess a lack of any Health&Safety law posters, a lack of health and safety information, warnings... all those kinds of compulsory warnings and rules displayed clearly. It shows that the workplace does not respect safety, follow safety laws etc. Such a place is likely to be unsafe.

Sloppy housekeeping is a good indicator. I'm an aircraft mechanic, and I once walked into a shop that was messy as all hell. I didn't bother to leave a resume because it was not the kind of place I would have felt comfortable working in.
