Should Simple IRA elective contribution percentage(and company match 3%) be calculated from initial gross salary of paycheck? My recent paychecks have this percentage calculated from gross after health premium and FSA have already been subtracted.

    No: AFTER since the health and FSA are pre=tax, they reduce your taxable income! The way it is being done is correct.
