I once ordered a meal which was about $15 and she brings out a plate of lobster complete with everything. I chowed down (The cost wouold have been $45) and had a couple of drinks.
Went to counter, paid the $15 and then hit the waitress with another $10 and told her I appreciated the error! (BURP!)

I would not send something back and have them re-do it (they might spit in it) but if something is unsatisfactory, I stop eating it and ask that it be taken off the bill.

Yes!If it was the wrong food...And I had to send back boiled shrimp once because it looked dryed out and actually stank!!The waitress said they were told to mix the old with the new,so it would do no good to replace it.I did not want to eat at all after the plate was sat before me and the smell hit the nose.My husband also lost his appetite.They didn`t charge us and offered to get him a doggy bag.But we left the food there.And it was a place that is known and visited from long distances away!

Yes most definitely, if it is not what I asked for, undercooked, etc, I send it back. If I'm paying for it, why should I have to eat an unsatisfactory meal?

If I receive the entirely wrong food, I send it back. Maybe if it's the middle of a rush and I wouldn't get the right food for a long time, and the food I was given looks good enough, then I'd keep it, but otherwise I'd send it back.
If I receive the food and there's some little problem with it, like there's mushrooms when I asked them to hold the mushrooms or they forgot the extra bacon I ordered, in that case I generally just scrape off the mushrooms and forget about it. I don't have any allergies.
I don't think it's appropriate to send a dish back just because you don't like it. You run an inherent risk when you order food at a restaurant. If your meal arrives and it wasn't how you expected it, that's your problem. You could have made your own dinner at home, but you chose to come out. It's basically gambling: you don't go to a casino and say you want a do-over when you lose your money. Even if it's so terrible in your opinion that you can't eat it at all, that's your problem. What can you do? You can pay your bill and tip and never return to that restaurant, that's what you can do.

If they brought me something wrong and it looked good, I might eat it. If they had to make me wait another 20 minutes because the waitress or chef made an error I might try to negotiate for a discount and keep the incorrect food.
If it didn't taste good (like spoiled) I would nicely explain that something was not quite right and order something else that is "safe".
I've been a waitress before and I'll tell you the truth now, if you ate where I worked I pissed in your food regardless, I'd just do it more if you were a minority.
