I have never faked it. I wouldn't.
But a couple of (naive or stupid) men have assumed that I have orgasmed through intercourse - without foreplay - and I just gave up on them rather than disabusing them. I figure either their previous partner was a woman who orgasmed very easily through intercourse alone (if I am feeling generous) or that they are selfish lovers and that wrapped up in their own pleasure that they don't bother to notice their partner's responses and don't want to believe they haven't orgasmed so easily because that would entail putting more effort in.
As for if faking is obvious... well there are so many physical indicators of orgasm - flush, nipple changes, vaginal contractions, breathing etc - and not all of them can be faked. I think a man who is in tune with your body or who actually understands women's responses would know when someone was faking. My boyfriend always says I couldn't fake mine if I tried.
