Tell him everything, its better than an e-mail that leaves the person with questions they might want to confront, this way they know why and the door is somewhat closed.
Short and sweet. Any reasons will only give an opportunity to open a dialogue to contradict.
I would tell him the truth and let it all out to him. Tell him what he is doing wrong and if that doesn't help then find ways to help you get rid of him.
Tell him why only if you think it will help him in the long run
