Foul balls are considered strikes. I don't know what they do with the hit balls. Maybe they're not counted at all. Good question though.

In MLB, how much time does a manager have to call in a relief pitcher when making a pitching change? Is there a penalty if he goes over?

You're reading Baseball broadcasters say, "By now Jones has thrown 40 strikes and 20 balls" or whatever. Who decides? Many pitches are never called ball/strike, such as hits. And who says whether a batter's 3rd - 4th - 5th fouls were in the zone?
