Actually, he doesn't have sex with them as regularly as he might have say... 20 years ago. I am sure that he HAS had sex with all of them, but he once said on TV that he was too old to fulfill the sexual needs of such a large group of women. There is also this I found online.

Playboy founder Hugh Hefner has revealed that his once active sex life has been replaced by a love of dominoes.
The 80-year-old maverick, who celebrated becoming an octogenarian in style earlier this year by extending his festivities between April and May, told The Sun that he no longer had the energy to indulge in his once famed antics.
He also told the paper: "I'm bored of the hanky panky. I'm still active but it's different – I like to play dominoes with my girlfriends."
