If the pitcher has engaged the pitcher's plate in the windup position, it is a balk if he moves to the stretch without legally disengaging first.
"Rule 8.01(a) Comment: From the Windup Position, the pitcher may:
...(3) disengage the rubber (if he does he must drop his hand to his sides).
In disengaging the rubber the pitcher must step off with his pivot foot and not his free foot first. He may not go into a set or stretch position—if he does it is a balk."
Not having checked the rulebook, I believe:
1. before stepping on the rubber, I don't think the pitcher can "balk"
2. once on the rubber, but not having made a pitch-to-the-plate motion, the pitcher may, without balking,
a. step off
b. step off and fake or make a pick-off attempt
Dropping the ball, even accidentally, is a balk.
3. once on the rubber and having begun a pitch-to-the-plate motion, it would be a balk to not complete the pitch to the plate.
