yes, and many still do. the best example would be Afghanistan. the country as a whole has never been united under one ruler or political leader. that is why it is one of the few countries that has never been conquered by foreign invaders, there has never been a central government that ruled all of it. as of now it is just a group of loosely knit tribes, ruled by tribal rulers.

NO. Someone has to be the leader at least enough to name the country. Once they get a taste of the power they want more.

Not necessarily countries as such but there are examples. Although i am far from an expert.
    eg
    -Icelandic Commonwealth (930 to 1262)
    -Gotland and Frisia (1350 - 1520)

Parts of the united states at various times have been run as Anarcistic communities, for example parts of Rhode island have been at some points set up as grassroots towns by anarchists.
During the Spanish civil war several areas of spain were functioning anarchist communities.

I am of the opinion that the only reason these places ceased to be anarchist, was because of the influence and somtimes violent takeovers by other nations/states and the personal agendas of the invading parties. Not as many would believe, the failings of the political system itself. (Particularly the main two examples i have given)

There have been societies with anarchistic elements or short lived sorts of societies but regardless of the answer to your question, it has no bearing on the validity or relative "success" of such a society. It has little bearing upon the possibility of anarchistic society. I might remind anyone reading this, that when the United States government was formed it was argued that such a government did not exist anywhere in the world and had not in any degree for ages.
