This took a while, but I think I have all four... Stanford University - Jim Plunkett (Raiders, won SB XV and XVIII), John Elway (Broncos, won SB XXXII and XXXIII) and Herbert Hoover (President 1929-33) University of Michigan - Tom Brady (Patriots, won SB XXXVI, XXXVIII and XXXIX) and Gerald Ford (President 1974-77) University of Miami (Ohio) - Ben Roethlisberger (Steelers, won SB XL) and Benjamin Harrison (President 1889-93) and I hope this one counts, I'm not an expert on the US college system so I might be wrong and this isn't counted as a college, but... US Naval Academy - Roger Staubach (Cowboys, won SB VI and XII) and Jimmy Carter (President 1977-81)
