Yes, plenty of times, but the one that comes to mind right now is Michael Gambon's horrendously bad portrayal of Albus Dumbledore in the last two Harry Potter movies. Personally, I think his portrayal of the great Wizard is abysmal, possibly because he has admitted to having never read a Harry Potter book in his life. Dumbledore all of a sudden became someone who was agressive with students, grumpy, angry, and generally pretty unkind. I have always thought that Sir Ian McKellen would have been an amazing choice - the perfect Dumbledore. But, perhaps he was loathe to take on another role where he plays an adored Wizard, or, it wasn't offered to him. Either way, I think he'd be fantastic.

Nicholas Cage in almost any movie, but particularly in The Weatherman, Kevin Spacey would have been better casting.

in star wars episode 2 and 3 .... for the part of anakin i would take away Hayden Christensen and replace him with Joshua Jackson, who was considered for the role and i think would have played "angry" and "evil" much better and would have come off less whiney

Yes, all the time. For what its worth I always think Woody Allen should play himself in his movies. :P

"The Green Mile" - - replace Tom Hanks with Ed Harris. (To me, Hanks is by far the weakest part in that very good movie.) +5
