I keep it as intact as I can. I hate folded pages. Books are awesome, so I treat them well, even the ones I might not enjoy so much.

As much as I like keeping books in great shape, I keep falling asleep on my books while reading in bed and I must roll all over them because every morning the book looks worse.

I tend to keep most intact. If I really like it, I buy a hard cover edition to keep on the shelf. Self help books I tend to underline, highlight, dog-ear, everything I can do to remind me of salient points EXCEPT tear out pages and keep a scrapbook of 'em.

I do both...it depends on the book and the reason for reading it. Like a novel, I just read it and keep the book nice. Should it be a study type book or self help etc then I underling or use high lighter for the important points beint made.

I underline and fold pages so I can go back and re-read any passages I like. Someday I plan to have two small libraries in my house. One will be full of the books I've underlined, and then I'll get hardback copies of the same books and let that be my "clean" library.

I make notes and draw pictures in my books. I make it personal, and it feels like it has become part of me. Then I look back at what I drew to try to figure out what I could POSSIBLY have been thinking. I do slide the covers off and store them though.

I keep HB first editions as nice as possible but paperbacks get the fold, underline and margin-writing treatment as do books for school or reference.

I prefer to keep them as nice as possible, except school books which get full highlight, underline, notetaking treatment. I try to avoid dog-earing though and prefer to use bookmarks or those little sticky flags.
