You want to ask if you can take the car to your own mechanic for an evaluation. Yes, you will have to pay a mechanic for his/her time but it will be worth it not to get stuck with a bad car. OR, if is from a car dealer, what kind of warranty is being offered if any.
look under the car, check for rust.. rocker panels can be a pain to get fixed. If you know what car your going to look at, check online what usually goes on them. If possible bring a guy, they tend to get yipped less often buy seller even thought society is more modern now than ages before.
get them to sign something that says ' car being sold under the premise that it will pass e-testing or safety'
Find out if it was ever in an accident. In some places you can get the car's history. You go to the license bureau and request it, cost is about 20$. But like mailpup suggested, get a mechanic to check it out for you. Oh yeah, and don't ever pay the asking price, unless the dealer is a "no dicker" dealer. I just bought a car last month, and got the price reduced by 500$ AND got them to pay the taxes as well.
