• They had syphilis back then. It was largely incurable and sometimes fatal in the 20s.

  You are right. They hadn't synthasized penicillin then.

• They discovered penicillin in 1928 and shortly thereafter figured out that it was a great treatment for syphilis, even in the advanced stages. However it was some time before there was a definitive test for the disease, and before that development, it was a maddeningly difficult disease to diagnose. It presented symptoms that varied and mimicked a whole host of very different disorders.
