What kills you from exhaust of a car is Carbon monoxide. It is very lethal and in non-odourous and colourless gas.
It is usually a product of incomplete combustion. If combustion is good (complete) carbon-di-oxide gas is formed, which is not lethal but asphyxiates if present in large quantities.
As amount of Carbon monoxide produced by smoking is very small compared to exhaust of a gas, in enclosed spaces car exhaust becomes lethal.
