Anyone who creates a file, or writes a program that creates a file, has full control over the extension of the file. The operating system keeps a list of extensions, and the programs that are associated with them. Some extensions mean specific things to the operating system, like .EXE would indicate an executible file. Some extensions are relatively cross-platform and/or software independent, such as .MP3. Some extensions have multiple software vying for their use, such as .DOC. When a program gets installed, it can set up these file associations. The reason Word opens a .DOC file when you double-click on it is because Word associated itself with .DOC files. The reason one particular player opens .MP3 files is because it associated itself with .MP3 -- it is possible to have more that one program that could open a particular file.
