It just means a specialist in something that is based in a central office but travels to different communities so that he or she can be closer to clients. Eg if they are an debt advisor for an advice centre they will be based at the centre but might do attend appointments or run drop ins at different venues in the community eg one day they may visit a hostel and the next day they may visit a university. Sometimes they might visit the clients in their own home. They are often concerned with health eg at the local health centre there might not be a gynaecology specialist based there but a community outreach worker who works in gynaecology based at the hospital may arange to see some clients at the local health centre.
