A blouse most commonly refers to a woman's shirt, although the term is also used for some men's military uniform shirts. Blouses are often made of cotton or silk cloth and may or may not include a collar and sleeves. They are generally more tailored than simple knit tops, and may contain "feminine" details such as ruffles or embroidered decorations. A shirt is a sort of top, i.e. a piece of clothing for the trunk of the body. In the UK, it refers almost exclusively to what Americans call a dress shirt, i.e. a garment with a collar and a full vertical opening with buttons. A tailor would be the right person to tell the difference. A shirt is somewhat loose fitting so the cuts and stitches in the cloth are in somewhat straight lines. Easier to satisfy the customer. But in case of a blouse cutting the cloth to measure and stitching it with all contours to fit correctly to the satisfaction of the customer is a lot more difficult. And they charge more to stitch a shirt!

They're pretty much the same thing to me. But obviously a blouse is for women. Then again, I call my school shirt a shirt. But I suppose that could be a blouse. I know in America 'shirt' can be like a tshirt, too. Or like a top, I think. But in the UK, shirt only means a proper shirt. I'm not getting anywhere with this answer...
