Depends on the Director of the film. If the director is a perfectionist the exact emotions would have to be expressed in the face of the two having sex as in a real life situation. This can be done only if they were actually having sex. The perfectionist Director will also keep directing the actors during the sex act about the exact body movements and expressions. There will be retakes if the director is not entirely satisfied with some asprect the previous shoot of the sex act. Should be very tiring and exacting for the actors and the technicians to work with a perfectionist film Director.

Very rarely. I read once that while shooting a sex scene the actors felt like actually doing it. So they asked the director and cameraman to leave the room for some time. The filming continued an hour or so later. These things can happen and perhaps do more often than we think.
