If you've ever seen a pictures of various kinds of fetuses, you'd have noticed that in the beginning, most animals look alike. We all start out with about a dozen nipples, just like a dog or a cat. As the human fetus grows and develops, the extra nipples are reabsorbed. Sometimes they all don't get completely absorbed. i know three people with extra parts of nipples, not full blown nipples, they look more like moles. One is my daughter, she had it removed. A friend with a third nipple noticed it lactating a bit when she became pregnant.

My boyfriend has four nipples. Two are under-developed and are on his abdomen. He says every male in his dad's family has them. I guess it's a genetic defect. I think it's called "polythelia" when they are just the physical nipples. (and in the case of beenarountheblock: supernumerary nipples that actually have mammary glands are a condition called polymastia, just a little different) ( I am only answering the second question really, the first was pretty clear lol)
