Gutters definitely were the passage for dirty sewage and the deepest parts lining the sides of the cleanest of roads. If someone were to fall into the gutter he will not only leavig the clean roads and getting soaked in dirty sewage but will be going down to hit a new low. Probably the phrase was coined to imply the drunks who kept falling into the gutters who had already hit a low in their social standing. This must be the origin of the phrase.
