Every women are different. If the nipple starts getting hard its a good sign shes enjoying it. Me personally as a women, I like it when I'm laying in my husbands lap and he reaches over my shoulder and puts his hand in my shirt and starts caressing it. Then there is when he grabs the whole breast and starts massaging the nipple with his fingers. Ans sometimes just a gentle caress over the nipple will get things started. But just experiment, and it that don't work. Ask her what she likes.
