Okay... First save it to a temporary directory.

Close your mail client and any other programs you might be running.
.
Ensure that your anti-virus and registry-protection programs are up-to-date.
.
Use Explorer to open the temp dir and double-click the file.
.
Windows will prompt you to "search the web" for a compatible program.
.
Go to the page Windows indicates and pray that there's enough info to determine the originating program.

If that doesn't help, you could always e-mail the sender and ask them WTF?
