I think Helen Mirren could be called a handsome woman. In a woman, handsome means a dignified attractiveness.
Something like this:

I think she looks like this.

looks as good as a guy would! hehehehehe
I think that phrase has to do more with the facial features then the body of the women, sort of like a strong beautiful chiseled face more then a round "pretty" one!
A young Katheryn Hepburn comes to mind!

I think it may be age-related in 'Madison Avenue-think'. "Hot", then "MLF", then "handsome".

Because 'handsome' is usually defined as masculine, it implies that the female has masculine features, but not necessarily masculine LOOKING.
For example, a more prominent nose, defined jaw, etc.
Not as handsome as this "handsome guy"!!
