It comes from the European origins of the Mafia. The term "don" is used in both Italian and Spanish cultures as the definition of "Sir". Although historically this represented someone of knighthood, it gradually evolved into someone of high importance and respect.

Music executives prefer banking on safe bets and are more likely to find interest in bands already with a strong public following and previous sales. Kinda like the bank who will loan you money when you're no longer in debt.

Depends on the club and the DJ. Many DJ's prefer vinyl to CD for more overall control of the song's speed, timing of entrance and possible mixing.This is slowly being phased out, however, as the vinyl medium is slowly discontinued.

Do nightclubs have to pay royalties for each song they play?
Technically, they should as a paid performance as a part of the BMI/ASCAP royalty agreements but it is my experience having worked for nightclubs that the majority do not engage in this practice. Others may, but I have never personally seen this. I have also have never heard of ASCAP or BMI pursuing a nightclub for owed royalties, but this is my own experience.
