A flat open bridge is correct, though the thumb should still be "pinched" up to the index finger. Raise the butt of the cue by 5-10 Degrees so as the cue tip has a better chance of making a fuller contact on the cue ball correctly.
Playing flat to the table may result in more mis-cues than you would like
as the cue tip has less of the cue ball to hit.

Simple answer:
the object ball and cushion must be hit at exactly the same time. this applies to all angles exept dead straight! slight deviation will not pocket the ball, but the shot is not as difficult as it presents itself. you just have to judge the correct contact that will hit both the cushion and the object ball at the same time.
