The blanket prohibition on open toed shoes and pantyhose is silly.
That said, one has to be careful about the look, because it can frequently look bad. I don't wear open toed shoes with pantyhose myself, but that's because I don't have any open toed shoes at the moment.

I've smoked about 1/3 a pack of cigarettes a day from the age of 14 to now, at 39. I would like to stop,but it's a bitch! I've tried nicorette gum and hated it. What has anyone else done with success?
I removed myself from my stressors for a week and didn't bring any cigarettes with me. A week is al you need to break the nicotine addiction (it's a powerful, but short-lived, withdrawal).

You can never, ever go wrong with the Little Black Dress! That said, there is a difference in black tie events between those that permit cocktail dresses, and those that require a full-length gown. When in doubt between those two, overdress and go with the full-length.

The dress is too dark for black. I'd go with a silver or pewter. Perhaps even the right nude or tan shoe could work, but no black for that dress.

Practically speaking? Beyond the legal differences, very little. Psychologically speaking? There's a sense of commitment to marriage that's lacking in merely living together. I wear 4 inch heels and higher, and I find that they actually help my posture, because I have to stand up straight to stay comfortably balanced. I don't wear heels higher than I can walk in, so the highest I can go is 5.5". I can get that high because I'm 5'10", so it's not as big a deal for me as it would be for a woman who's 5'.

No, but I can wear 5" heels without having a hard time walking at all. 6" would be hard for me, so I don't bother with those. It helps that I'm 5'10", though, so 5" for me is a lot less of an extreme angle for my foot than 5" for a girl who's 5' even.

Department stores, or online. I wear a size 10, and I don't have trouble finding awesome, gorgeous, cute heels whenever I want. Personally, I love them!
The width might make it somewhat more difficult, but all the more reason to go to a high-end department store (to help with the fitting) or online at a place like zappos.com (for the mind-boggling variety they carry). Good luck!

Silver or pewter would be very nice. Don't write off silver as too ordinary based on the color; you can find some drop-dead gorgeous, bright, sexy shoes in silver that would be perfect with a teal dress. You just have to look at the style.
